#PIHealth: A Call to Action for Improving Health in the Philippines

Note from the editor: Today's story is a guest post by Kathleen Cabangon. A major issue that the Philippines faces is health and healthcare. Around one-quarter of families in the Philippines live below the poverty threshold, reflecting broad social inequity and other social challenges. Cities are becoming more crowded and polluted due to increased migration from rural areas; cost of living has increased due to inflation making food more expensive; and access to clean water supplies is more uncertain compared to a generation ago. Despite the Philippines’ economic and political progress in the new millennia, inequity in the public health sector exacerbate challenges in morbidity and mortality and put at risk the well-being and lives of many Filipinos.

Acknowledging the growing concerns in health, the Philippine government is expected to provide universal health care for its citizens by 2016. An article published earlier this year by the World Bank, “Achieving Universal Health Care in the Philippines,” described the current state of the health care system:

“While the Philippines has made considerable progress in the health sector over the previous decades, several important challenges remain. Poor households’ health outcomes were three to four times worse than middle class families’ and the poor lacked proper financial protection from debt accrued from out-of-pocket health expenses. To alleviate poverty, the government prioritized enrolling poor households in the national health insurance program and ensuring they had adequate access to quality health services and financial protection.”

My interest in health issues began while I was an intern at Advancement for Rural Kids (ARK). ARK developed an amazing feeding program: just $15/day feeds one child for an entire year. This program was backed up by statistical data which showed the shift. With this, I began to draw the conclusion that without proper access to food or health care, success was virtually impossible. For example, imagine all the times you were sick and unable to work. Being ill essentially put a “stop” on your ability to produce and be active. There is a direct correlation between productivity and one’s state of health. We can further push that idea to say that for some in the Philippines, the notion of productivity becomes an obstacle because they do not have the necessary health care to remain healthy. Personally, I feel an inherent need to give back in any way that I can to the Filipino community and one way I fulfill that need is by writing this blog post. My role is to give a voice to the people who are voiceless and to expose the hardships Filipinos face, day in and day out. It is a shame that even for a nation with infinite potential like the United States, that inequities such as poverty and access to health are still unsettled issues for the country. Public hospital facilities should be beyond what they are now and should mirror the city’s aims towards growth. The prices of medicines in the Philippines are among the highest in the world—certainly too high in relation to household incomes of most Filipinos. Given the high prices, most medicines are beyond the paying capacity of most Filipinos. Thus, the state of health in the Philippines calls for attention from the government, but most importantly from its citizens, where citizen awareness of the issues is the primary catalyst for change.

What I Learned About Spirituality from My Grandmother

One of the first things I learned in life was how to pray. My grandmother, who was my main caretaker, taught me the basics of the Catholic dogma as soon as I could speak: the Lord's Prayer and the Hail Mary. As I got older, my parents enrolled me in a Catholic school where I spent ten years of my young life, mostly in rebellion against the very practices I was being taught. My mother forced us all to go to church on Sundays, which I despised, on top of going to evening confirmation classes in my last year at the Catholic school. I felt at the time that religious practice was filled with hypocrisy. I would see families gathered together in a forced fashion for an hour, much like mine, pretending to be devout followers of the lessons of peace being preached, only to be cursing at other drivers trying to leave the church lot in haste. An hour in the church did not feel like it was enough to be genuinely in touch with your higher power. It bothered me. I began studying other religions and modes of spirituality in my teenage years. It wasn't the idea of God that I was against, but I needed a practice that would work for me. I always knew that the Philippines’ deep-rooted Catholicism came from its colonial history with Spain, but I began to wonder what came before it. I looked high and low for what Filipinos may have practiced before Catholicism, but I didn’t come up with much. In the meantime, I became very attracted to Pagan spirituality for its secrecy, its mystical nature, its spiritual practices that didn’t seem forced and its rituals that were not bathed in pageantry.

But all along, if my grandmother invited me to pray with her at home, I never hesitated to join her. She is a deeply devout Catholic, yet something about her spiritual practice resonated with me. She never questioned my rebellion or reminded me about church and because of that, I never argued with her about spirituality. Instead, I followed her lead. She would offer simple wisdom in Tagalog: “If you feel troubled, talk to God. Come and pray with me.” I would sit quietly with her and watch her light a candle, take a rosary in her hands and close her eyes. She said nothing aloud, nor forced me to say anything. It was beautiful to just watch her, and wonder what she was thinking, or saying to God. I couldn’t help but notice how similar her practice was to some of the Pagan rituals I'd researched.

Later in life, no matter how much I tried to deny my religious upbringing and break away from my family’s spiritual identity, I found myself practicing the same things as my grandmother. Away from my family in college and confronted with bewildering new situations, I began trying to talk to God on my own. I stopped going to the church, but my practice continued, quietly, in the peace and privacy of my own mind.

Nowadays, I don’t subscribe to any religion, but I feel I’ve found my spiritual practice, and it takes the same form as the practice my grandmother and I used to do together at home: a quiet meditation and a one-on-one conversation with the powers that be. It’s helped me get through a lot of difficult situations when I've felt that there is nothing left for me to do but go back to that quiet place in my mind and heart. I no longer feel like I need to rebel against Catholicism and the Church anymore - I see that it enabled me to seek my own truth and opened me to the possibility of something beyond what's tangible and logical. I understand now that spirituality is more than just a religion, but something deeply personal that transcends man-made rituals and practices. It’s incredibly life-enriching to find and create a spiritual practice, if you can allow your mind and heart to go there.

UniPro's The Vagina Monologues: Breaking the Maria Clara Image, Indeed

Moans, writhes, orgasms … it wasn't your typical UniPro event. On March 9, UniPro hosted The Vagina Monologues: Breaking the Maria Clara Image at Cap21 Studios. Based on interviews with real women, The Vagina Monologues is a play by Eve Ensler featuring hilarious, heartbreaking and uncomfortable confessions from different women about their, well, vaginas, relating to their personal stories of femininity and sexuality.

UniPro’s The Vagina Monologues production starred an all-Pilipina cast, with a special Pilipina twist added to parts of the script. In one monologue, for example, a “lola” ashamedly discussed her “down there.” Then at the end of the play, the cast stood side by side onstage, taking turns to share disturbing facts about victims of sexual violence:

"One in three women on the planet will be beaten or raped in her lifetime. That’s more than one billion women living on the planet today."

"The NDHS revealed that one in five women aged 15-49 has experienced physical violence since age 15."

"One in ten Filipino women aged 15-49 has experienced sexual violence."

Following the play was a panel of representatives from various women’s rights organizations. These distinguished women included Ivy O. Suriyopas, Director of the Anti-Trafficking Initiative at AALDEF; Kristina K. Joyas, a member of AF3IRM (and UniPro’s Director of Staff Development); and Zarah K. Viñola, Vice-Chairperson for FiRE. They discussed ways their organizations are tackling issues that affect Pilipinas, as well as their own definitions of the term “feminist.”

Confronting topics ranging from rape and sexual violence to self-image and self-discovery, the night was emotional and thought-provoking. It was a seamless event and production, organized by Kirklyn Escondo, our Community Building Director, and directed by Precious Sipin and Leslie Espinosa. Music also added to the drama of the play, with Andre Ignacio Dimapilis on the didgeridoo and Andy Jean-Gilles on the djembe drums. Lastly, Stella Ma also spoke on behalf of the NYC Chapter of the National Pacific American Women’s Forum, informing the audience of the recent publication of their Health Resource Guide.

It’s rare seeing Pilipinas onstage, portraying characters with real depth to whom we can actually relate. It’s a stark difference from the roles Asian Americans are usually degraded to: the token Asian friend, unnamed nerd or exotic lover. Let’s not forget the title of our production, which references Maria Clara, the iconic character from Jose Rizal’s Noli Me Tangere. This tragic heroine is known for her sweetness and obedience; she is a symbol (or perhaps a caricature) of the ideal Pilipina. Well, with all the talk of vaginas that Saturday night, the strong and talented women of UniPro’s production of The Vagina Monologues couldn't be any farther from Maria Clara.

Albularyo Advice

The worst of flu season may soon be over, but it’s always good to keep some natural remedies in mind year round. When western medicine will only do so much, I like like to think of the albularyo remedies I've been taught through the years. The Tagalog word “albularyo” is used to describe pre-colonial medicine men and women of the Philippines. The word may be derived from the Spanish word for herbalist. The Albularyo would combine spiritual practices with natural concoctions to heal people in need. When it comes to keeping my health in check, I like to think like an Albularyo and return to the fruits of the earth for medicine. One potent wellness ingredient that is found in a lot of Filipino dishes is fresh ginger. This warming root can be boiled raw, and the resulting "tea" can be consumed for a natural digestive aid that also acts as an anti-inflammatory medicine. It actually helps to ease muscle aches from the flu and even arthritis. It is also a great natural immune-system booster. One dish that is heavy on ginger is arrozcaldo - that can explain why this simple and yummy comfort food always seems to work miracles when you've got a bug!

Another common and powerful herb commonly used in Filipino dishes is garlic. It is a powerful anti-inflammatory and a natural antibiotic. One treatment of the common cold is to eat a clove of garlic raw. If that’s something you’d rather not endure, you can boil 2 cloves of chopped fresh garlic together with one lemon and a dash of cayenne pepper. Strain the water after boiling and add honey to taste. It’s a bit of an acquired flavor, but the garlic, together with the vitamin C from the lemon, the congestion-busting heat of the cayenne pepper and the natural antibiotic and throat-soothing properties of honey make this a formidable treatment against the common cold.

For daily wellness, I like to start my day with a green smoothie. There are many recipes out there - all you need is a powerful blender. A dose of fresh greens and sweet fruits a day blended in a tasty drink will provide you with tons of vitamins, minerals and antioxidants to keep you energized and well. For the cold days, I like to blend together spinach, fresh pear, a banana, a dash of cinnamon and vanilla soy milk. Heat it up in the microwave for a nice, warm, creamy drink. Don’t knock it ‘til you try it!

Behind Closed Doors: Fil-Am Mental Health

Mental health and safety aren’t regularly discussed in our society. Our culture as a whole has stigmatized mental illness, as if it were an infectious disease. In a recent report, the National Alliance on Mental Illness (NAMI) notes that the US mental health system is far from sufficient. Health services and laws vary across states, further deterring individuals from seeking help. However, in light of recent mass shootings, discussion on mental health in America is becoming more prevalent. But why must it take a tragedy to discuss something so important? The Fil-Am community, which has more easily assimilated into American society in comparison to other Asian communities, rejects the help-seeking mentality. In a study, E.J.R. David says that “cultural mistrust” plays a major role among the Fil-Am community. Mistreatment and oppression throughout Pilipino and Fil-Am history has certainly influenced this mistrust. However, one of the findings was that younger generations are more inclined to seek psychological help, as they don’t have the same concerns as first generation Pilipinos. Those more assimilated and familiar with American culture were more willing to seek help. Furthermore, the study found that those with a higher economic status have more opportunities to seek help.

Like in other Asian communities, loss of face and shame are feared among Pilipinos. As a young Fil-Am, I unknowingly lived with this mentality. I used to be afraid to open up and ask for help when I needed it. Sharing my most difficult struggles wasn't originally part of my personality, nor my identity. For me, admitting that I needed help required a lot of courage and a loving support network. It wasn't until my second year of college that I realized seeking help was even an option. After a referral from one of my advisors, I visited the William & Mary Counseling Center. I was afraid; I felt like I was admitting defeat, and that I wasn't strong enough to face my problems. I eventually realized that I was wrong. I ended up returning to the Counseling Center throughout the rest of my time at William & Mary. I attended meditation sessions, individual therapy, and, in my opinion the most helpful, group therapy. Being able to speak my mind, without fear of judgment or having to follow through with explicit advice, was reinvigorating. I knew I was being heard, and in turn, got as much out of it as I put into it.

After such a positive experience at the Counseling Center, I was proud of my newly-acquired love for seeking help. I found myself encouraging others to also take time out of their day to explore new outlets, be it exercise, lost hobbies, or seeing a therapist. I even got involved with student government and organizations on campus in order to spread mental health awareness.

I only hope that the younger generations can embrace this mentality, and encourage their peers and the larger Fil-Am community to do the same. We can only benefit from confronting Fil-Am mental health. Regardless of cultural expectations, it is important to be selfish when it comes to your personal well-being. Speaking from personal experience, I assure you: there is absolutely nothing wrong with asking for help.
